Body Mass Index, or BMI, is one of the most widely used health screening numbers in the world โ it appears on medical forms, insurance assessments and fitness apps. It's also one of the most widely misunderstood, often treated as a precise, individual verdict on someone's health when it was designed as a simple, population-level screening tool.
What BMI actually measures
BMI is a ratio of weight to height, calculated as weight in kilograms divided by height in metres squared (or an equivalent formula using pounds and inches). It was developed in the 1830s by a Belgian mathematician, Adolphe Quetelet, not as a medical diagnostic tool but as a way to describe the 'average man' across a population for statistical purposes. It only became a widely used individual health screening measure in the 20th century.
The standard categories
For adults, the commonly used BMI ranges are:
- Below 18.5 โ underweight
- 18.5 to 24.9 โ normal weight
- 25 to 29.9 โ overweight
- 30 and above โ obese
These thresholds are population averages associated with statistically higher or lower risk of certain health conditions, not individual cut-offs that apply identically to everyone.
Why BMI has real limits
BMI's biggest blind spot is that it cannot distinguish between muscle mass and fat mass โ it only measures total weight relative to height. This means:
- Athletes and very muscular people often score in the 'overweight' or even 'obese' range despite having low body fat, because muscle is denser than fat.
- Older adults can have a 'normal' BMI while carrying a higher proportion of body fat and less muscle than a younger person at the same BMI, since muscle mass naturally declines with age.
- Where fat is distributed matters for health risk (abdominal fat carries different risk than fat elsewhere), and BMI doesn't capture this at all.
- BMI ranges were developed primarily using data from certain populations and may not translate identically across all ethnicities, which is why some health bodies use adjusted thresholds for specific populations.
So is BMI still useful?
Yes, with the right expectations. BMI remains a fast, free, and reasonably useful screening tool at a population level, and for most people without unusually high muscle mass, it does correlate loosely with body fat and certain health risks. The key is treating it as a starting point for a conversation, not a diagnosis โ a single number that might prompt further investigation (like body fat percentage, waist circumference, or blood tests), rather than a conclusion on its own.
